You eat, and twenty minutes later there is burning behind your breastbone. You have been told the problem is too much acid. Acid gets suppressed, some of the burning settles, and yet the bloating, the belching, the fullness and the sense of food sitting like a brick often stay exactly where they were. That combination is the tell.

Read that as a gap in the explanation, not as a verdict on your treatment. If a doctor has put you on acid-suppressing medication, that decision is theirs, it was made for reasons that matter, and it stands. Nothing below is a reason to change it. What this article is for is the part of the picture the too-much-acid story leaves out, because burning tells you acid is in the wrong place. It does not tell you there is too much of it in the right place.

## Key takeaways

- The valve at the top of your stomach is closed by acidity, not by the absence of it.
- Fermentation in a slow, weakly acidified gut makes gas, and gas makes upward pressure. Pressure is what pushes stomach contents where they do not belong.
- You cannot make stomach acid without adequate zinc, so a mineral gap upstream can show up as burning downstream.

## Why Do You Get Heartburn When Your Stomach Acid Is Low?

Start with the valve.

The lower oesophageal sphincter is a ring of muscle at the top of your stomach. Its job is to stay shut while the stomach churns and to open only when you swallow. It is not a passive flap. Its tone is regulated, and one of the things that regulates it is how acidic the stomach contents actually are.

Adequate acidification of the lower stomach is part of the signal that tightens that ring and tells the stomach to move the meal along. Weak acidification is part of the signal that lets it sit open more often and for longer.

So low acid does not protect you from reflux. It removes one of the things that was keeping the door shut.

And what comes back up when the door opens is not pure acid. It is a mixture that still contains pepsin, the protein-digesting enzyme, and very often bile that has travelled the wrong way from the duodenum. Your stomach has a thick mucus layer built to survive that mixture. Your oesophagus has almost nothing. It burns whether the pH is 1.5 or 4.

That is the part nobody explains. The severity of the burn tells you about the tissue being exposed, not about the volume of acid your stomach produced.

## What Does Stomach Acid Actually Do Besides Burn?

Acid is a signal as much as it is a solvent.

It denatures protein so the structure unravels and enzymes can reach it. It converts pepsinogen into active pepsin, which only works in an acidic environment. It sterilises the meal, killing most of what arrives on your food before it reaches the small intestine. It triggers the release of bile from the gallbladder and enzymes from the pancreas. And it helps set the pace at which the stomach empties.

Take the acid down and every one of those jobs runs at half power at once. That is why [low stomach acid and poor bile flow](https://gutfix.ai/blog/mediterranean-diet-bloating-bile-stomach-acid) show up together so reliably. Bile release is downstream of acidity. The first domino sits at the top.

This is the gut cascade in order. Stomach acid, then bile flow, then the oxygen gradient in the small intestine, then the mucus lining. Symptoms are downstream of whichever layer failed first, and heartburn is one of the few symptoms that sits almost on top of the first layer.

## Why Does Heartburn Come With Bloating and Belching?

Because they are the same event, seen from two ends.

A meal that is not properly acidified sits in the stomach longer than it should. Delayed gastric emptying means a full stomach for hours after eating, which is the heaviness people describe as a brick.

Food that sits ferments. Bacteria that adequate acid would have handled survive the passage into the small intestine, where the population is meant to be sparse. They meet undigested carbohydrate and they do what bacteria do with sugar. They make gas.

Gas in a closed abdomen has to go somewhere. It raises intra-abdominal pressure, and that pressure loads the stomach from below.

There is a second consequence worth naming. The small intestine is meant to stay low in oxygen and low in bacterial numbers. Once acid stops filtering what arrives, and once the meal is loitering, that oxygen gradient shifts and the organisms that thrive in the changed conditions take hold. That is the terrain shift behind [most of the triggers for overgrowth](https://gutfix.ai/blog/what-are-the-triggers-for-sibo), and once it is established the gas production becomes a daily event rather than an occasional one.

Now put the two together. Raised pressure underneath, a sphincter that is holding less tone than it should because the acidity signal is weak. The mixture goes up.

That is why the burning is worse after a large meal, worse when you lie down, worse when you bend forward to pick something up, and worse with a tight waistband. All four of those raise the pressure difference across the valve.

It is also why so many people find that the timing of their bloating is diagnostic in the loose sense. Bloating that starts during a meal or within the first hour points upward, toward acid and emptying. Bloating that builds hours later points further down the tract, toward [fermentation and motility](https://gutfix.ai/blog/how-can-i-improve-my-gut-motility).

## Can Posture and a Hiatus Hernia Cause Reflux?

Yes, and this is the part where no supplement helps.

The lower oesophageal sphincter does not work alone. The crus of your diaphragm wraps around the same spot and acts as a second, external sphincter. Two closures in the same place, working together.

A hiatus hernia is when part of the stomach has slid upward through that opening in the diaphragm. The two closures uncouple. You lose the pinch of the diaphragm on top of the muscle ring, and the pocket of stomach sitting above the diaphragm becomes a reservoir that refluxes easily.

Posture matters for the same mechanical reason. Slumped sitting compresses the abdomen and raises the pressure under the valve. Heavy lifting with a braced breath hold does the same thing deliberately. So do restrictive waistbands and eating a large volume shortly before lying down.

None of this is a character flaw and none of it responds to changing your diet alone. A hernia is a structural finding, and structural findings are assessed by a doctor with the right imaging, not by a questionnaire and not by us.

## Why Can You Not Make Stomach Acid Without Zinc?

Because of one enzyme.

The parietal cells lining your stomach do not store acid. They build it, meal by meal, from carbon dioxide and water. The enzyme that carries out that first step is carbonic anhydrase, and carbonic anhydrase is zinc-dependent. No zinc, no hydrogen ions, no acid at the volume you need.

Zinc does not stop there. Mucus production needs it. Antimicrobial peptides need it. The gut lining turns over quickly and that turnover needs it too.

So a zinc gap can quietly lower your acid output and thin your protective lining in the same stroke, which is the least helpful combination available. That is a mechanism worth understanding, not an instruction. What any individual should take, in what form and alongside what else, is a conversation with a practitioner who knows your full picture.

## What If You Are Already Taking Prescribed Acid-Suppressing Medication?

Then keep taking it exactly as your doctor prescribed it.

Do not stop a prescribed medicine because of anything you read here. Do not reduce it, delay it, swap it or come off it either. Acid suppression is prescribed for real and serious reasons, including erosions, ulcers, bleeding risk and protecting the oesophagus from lasting damage. Whether it continues, changes or ends is your doctor's decision and only your doctor's decision. If our thinking ever conflicts with your prescribed care, we change ours.

What we can do is work on everything sitting around the medication. Meal size and pace. How well you chew. Whether you flood a meal with liquid. How long you stay upright afterward. Waistband pressure. Bile flow and [the state of the mucus lining](https://gutfix.ai/blog/signs-of-poor-bile-flow). Motility, and whether bacteria are overgrowing where they should be sparse. Those levers belong to you, and they are the ones most people have never been shown.

Get urgent medical help rather than reading further if you have blood in your stool or black stools, unexplained weight loss, severe or persistent pain, difficulty or pain swallowing, vomiting blood, or a fever alongside your symptoms. Those are not gut terrain questions.

## So Where Do You Actually Start With Heartburn?

With the timing, not the burning.

Note when the symptoms hit relative to the meal, what the stool looks like, whether the fullness lands early or late, and whether posture and position change anything. That pattern points at the layer of the cascade that failed first, which is far more useful than another round of guessing at foods.

Our [gut type quiz](https://gutfix.ai/quiz) reads that pattern in a few minutes. It is educational and it does not diagnose anything, including reflux, a hernia or an ulcer. What it does is tell you which layer to look at first, so you stop treating a downstream symptom as though it were the cause.

The burning is real. It is just rarely the beginning of the story.

## Frequently asked questions

### Why do I get heartburn if my stomach acid is low?

The valve at the top of your stomach is closed by acidity, not by the absence of it. Adequate acidification is one of the signals that tightens the lower oesophageal sphincter and tells the stomach to empty. When acidity is weak the valve holds less tone and the meal sits longer, and a meal that sits ferments and makes gas. That gas raises pressure inside the abdomen and pushes stomach contents up against a valve that is already not sealing properly.

### If it is not acid volume, why does the burning hurt so much?

Because of what the tissue can tolerate rather than what the stomach produced. What refluxes is a mixture containing pepsin and often bile, not acid alone. Your stomach has a thick mucus layer built to survive that mixture. Your oesophagus has almost no protection at all, so the same mixture that the stomach shrugs off burns the moment it goes the wrong way.

### Why do heartburn and bloating happen at the same time?

They are the same event seen from two ends. A weakly acidified meal empties slowly, bacteria that acid should have handled survive into the small intestine, and fermentation produces gas. The gas is the bloating. The upward pressure that gas creates is what drives the burning, which is why both are worse after a big meal, when lying down, when bending forward and with a tight waistband.

### Can posture or a hiatus hernia cause reflux on its own?

They are genuine mechanical contributors. The crus of the diaphragm acts as a second sphincter around the same spot as the muscle ring. In a hiatus hernia part of the stomach has slid up through that opening and the two closures uncouple, so reflux happens more easily. Slumped posture, braced heavy lifting and tight waistbands all raise the pressure under the valve. A hernia is a structural finding for a doctor to assess, not something a diet change resolves.

### I take prescribed medication for reflux. Should I change anything?

No. Keep taking it exactly as prescribed. Nothing here is a reason to stop, reduce, delay or swap a prescribed medicine, and any change to it is your doctor's decision alone. The work that is genuinely yours sits around the medication rather than instead of it: meal size and pace, chewing, staying upright after eating, waistband pressure, bile flow, the mucus lining and motility.
